Transcript Requests

Online

You may order your transcript online through Docufide Secure Transcript™. It's easy, it's secure, and it's available 24/7. Docufide will send you an e-mail to confirm that the transcript has been sent.

In Person, By e-Mail, or By Fax

You may order your transcript in person by visitng the Counseling office.

The following information is needed:
  • Name - Please include the name you used while attending MV.
  • Date of Birth
  • Year Graduated - If you did not graduate from MV, please indicate that as well.
  • Current Phone Number - In case we require more information from you.
  • Signature - Authorizing rlease of information
  • $5 per Transcript - Cash or Check

New Enrollment

The following items are required for new students enrolling into Monte Vista High School:

  • Proof of Birth - Birth certificate or Passport
  • Immunization Records - California School Immunization Law for K-12
  • Proof of Residency - Parents and guardians must be able to provide proof of residency. Please see the section marked: "Residency Verification"
In addition:

If your student has completed one or more semesters of high school, you must bring an Unofficial Transcript from their previous school.
If your student is in Special Education, you must bring a copy of the most current I.E.P.
If your student is transferring during the school year, you must also bring the Drop/Withdrawal form from their previous school.

Parents/guardians must complete all required documents in the registration packet and submit the forms to the Registrar.

Note: Registration packets are made available after March 1st for the upcoming school year.

Residency Verification

Proof of residence is required at time of enrollment and must be verified by one item from each category below:

One Required from this Group:
  • Purchase Agreement or Escrow papers that show close of sale
  • Grant Deed
  • Property Tax Bill
  • Current Mortgage Statement
  • Lease/Rental Agreement signed by both parties, with name and address listed
Current Utility Bill from the One of the Utilities listed below:
  • Electric (PG&E)
  • Water
  • Garbage
Cell phone bills or those listing a P.O. Box are Not Valid residency verification documents.

Please note that all information provided by parents/guardians must be true and accurate.
Any documents provided that are incorrect or not true will terminate the student's enrollment.

Withdrawal Procedures

Students transferring from or leaving Monte Vista High School to attend another school must follow the withdrawal procedures. This procedure is initiated by the Registrar or Counselor and includes completing a Withdrawal Form and returning all text books.

Immunization records and final transcripts will not be released until a completed Withdrawal Form is returned to the Registrar.
